Day Support Strategy
To implement the Day Support strategy to people assessed as requiring day support.
To approve the day support strategy to ensure:- Providers offer person centred planning, keeping the individual at the centre of provision, including working with families to ensure quality outcomes which promotes independence, choice and inclusion. It is important that day support offers a variety of choice which includes participation in partnerships with local communities. The strategy includes focussing on establishing warm environments in the community and that they are aware of requirements of the individuals who use them. To ensure building based support is available for those with most complex support needs, reliant upon specific equipment, space and environments. Also that carers are supported to carry out their duties while safeguarding their own health, wellbeing and employment (where applicable) through our Carers Strategy.
Current in-house day provision offers Supported Employment, access to horticultural projects, young peoples services, pathway to Kershaw for older people all with the ideal of offering a variety of activities which embraces the 5 way to well-being, person centred planning whilst developing independent life skills. The strategy sets out priorities and monitoring of outcomes which enhances the existing service, clearly identifying goals and how these are measured for all day support.
The day support strategy gives purpose to day support and ensures accountability for providers whilst ensuring services are aimed at those most in need and where it is needed.
