Transition Flats

Brookfield centre works in partnership with Torus to manage the process of the transitional tenancies and referrals made.

There are three properties in extra care housing schemes in St Helens, providing an interim solution which gives people an opportunity to see if they can live independently in a supported environment. These properties can also be used when an individual's home environment is unsuitable in the short term and they need to temporarily relocate. The properties provide short-term placements to aid community placement and hospital discharge for people who are unable to return home or are waiting for adaptions following a life changing event.

The aim of the transitional tenancies is to strengthen the capability of community and locality teams to support individuals in their community by providing bed-based care for individuals that need more intensive support, treatment and assessment for a short period.

All planning for admission, treatment, care and exit from properties will be facilitated through the St Helens Locality MDT approach and led by the bed coordinator.

Each referral must have a defined exit plan and up to date assessment and care plan, which will be reviewed in two weeks of admission.

The properties are fully furnished and a care agency - Human Support Group - is on site to support service users with their care needs.
